Medical Professional Member Signup

I hereby apply for membership in the Chippewa Valley Health Cooperative and agree to abide by its bylaws and regulations. I am at least 18 years old. I attest that I hold a valid license to practice medicine in Wisconsin and anticipate providing medical services at the Cooperative’s facilities. I understand that my membership will be valid upon receipt of the required minimum contribution.
